Why Home Health Care?

There's no place like home, especially when you are recuperating from surgery or a hospital stay. Your own bed, your family and friends are familiar and comforting. Being in your own home gives you a sense of independence, privacy and control. Home care is a safe and economical way to receive the help you need.

How do I receive home care?

You, your family, your doctor, nurse, the hospital or nursing home may call our office at 970-928-0601. We will check with your physician to determine the type of care you need. If required, we will come to your home for a free evaluation visit. If it is determined that you would benefit from home health care and that you meet the eligibility requirements of your insurance, you will begin receiving home care, usually within 24 hours of the referral.

You will be notified of the cost of your care at the time you begin receiving services.

How do I pay for home care?

Medicare, Medicaid and most private insurance companies will pay for home health care. There are usually several criteria required:

  • You must be under the care of a physician
  • You must have a limited ability to leave home

The type of care you need requires the skills of a nurse, speech therapist or physical therapist.Our staff will help you check with your insurance company to determine if you are eligible for services. If you do not have insurance, home care may be an economical choice for care.

Our Staff

Valley View Home Health Services is staffed by a team of specially trained individuals who have the knowledge and resources to coordinate services for you. All staff members are carefully selected and competency tested.

  • Financial counseling to assist with concerns such as finances, living arrangements and family issues.

These are the services Med-Psych Healthcare Services can provide in the home.

Medical Skilled Nursing: Full medical services of traditional home health for treatments such as wound care, high-tech infusion, Foley care, diabetic management, skilled observation and assessment, and many other nursing procedures. Services rendered by Registered Nurses and Licensed Practical Nurses consistent with a treatment plan established by the patient's attending physician.

Psychiatric Nursing: Registered Nurses trained in psychiatric treatment care for patients with emotional disorders consistent with the physician's care plan. Medication monitoring and management and one-to-one intervention are some of the psychiatric nurse's duties.

Home Health Aides: Certified Nursing Assistants assist patients with activities of daily living and personal care.

Physical Therapy, Speech Therapy, Occupational Therapy: Licensed Therapists use therapeutic methods to promote rehabilitation. Treatments to enable or improve personal mobility, dexterity and communication following a health crisis episode.

Medical Social Services: Licensed Clinical Social Workers assist patients in solving problems that may arise through illness, including social, financial and transportation problems.

Private Duty Services: All services above may be provided in the home on a per visit basis or on an hourly basis. We even have Live-In's (only CNA's and HHA's) that can be placed in the home as needed. These services are generally paid for directly by the customer.

Temporary Staffing Services: We now offer temporary staffing services. We can arrange temporary staffing of nurses or home health aides. No hiring and firing, payroll worries, or worker's compensation issues. We will cover it all for a nominal, competitive rate.

Nursing Services - provides physical assessments, continuation of hospital treatments, medication administration and monitoring, home IV therapy, psychiatric nursing and many other services that require skilled nursing. Our nurses work closely with your personal physician.

IV Therapy - delivers high quality IV therapy services directly to patients in the comfort of their own home. We manage your care in the following areas: antibiotic therapy, pain control medications, hydration, chemotherapy, solumedrol infusions, IGG infusions, baclofen pump maintenance and refills, TPN.

Private Duty Services - The Private Duty Division of Home Health & Hospice Care provides professional Nursing and Certified Home Health Aide support to patients and caregivers throughout Southern NH & Northern MA. Staffing can vary from a minimum three-hour request for services to total around the clock care. Services are provided 24 hours a day, 365 days per year. Please call us at 423-0216.

Certified Home Health Aides - assist with personal care, dressing, bathing, transfers (from bed to wheelchair, for example), therapeutic exercises, feeding and other services, all under the supervision and guidance of a registered nurse.

Homemakers and Companions - provide assistance in household management including cleaning, laundry, grocery shopping, and preparing and serving meals.

Rehabilitation Therapies - maximize independence while planning and assisting with treatments. Licensed physical, occupational and speech therapists are experienced in state-of-the-art health care.

Behavioral Health Nurses - a specialized nursing track of psychiatric home care that focuses on integrating the physical, emotional and spiritual needs of the patient. Current areas of need include: anxiety, depression, behavior difficulties, memory disturbance, thought disorders. Specialty services: mental status assessment and evaluation, case management, medication management, phlebotomy, education of client and family, evaluation of substance abuse and further treatment when needed.

Pediatric Services - deliver quality care and health education to the very young and their families in the comfort of home. Services include early intervention program, pediatric nursing specialists, physical therapists, speech therapists, occupational therapists, social workers, nutritionist, monthly well child clinics (for parents with financial hardships).

Maternal Child Health Nurses - respond to the special needs of families. Services include prenatal monitoring, newborn assessments and teaching, breast feeding support, SIDS counseling, care of the acutely and chronically ill child, well child care, immunization and dental clinics, intervention and home rehabilitation programs.

Medical Social Workers - respond to the personal, family, financial or work problems associates with the stresses of illness or disability.

Nutrition Counseling - includes counseling for new mothers and the elderly. A registered dietitian assesses and plans for a patient's nutritional needs, from basic meal planning to complex therapeutic diets. Evaluated whether enteral/parenteral feedings are adequate to meet patient's nutritional needs.

Hospice Care - provides the specialized services needed by patients and families facing the challenges of terminal illness. Working with the patient, family and the patient's personal physician, Hospice develops an individualized care plan and then mobilizes all of its resources to put that plan into action.

Grief Support for Children & Adults - services are available for Hospice families and individuals grieving a death. Grief Support Groups are offered to children (ages 4-12) and to adults. Individual counseling is also available.

Spiritual Care - provide patients and family members resources that can strengthen their ability to cope with illness and death. The spiritual component facilities understanding, meaning and compassionate listening around emotional and spiritual pain that often enables healing relationships for the patient and families.

Community Clinics - reach out to the community-at-large as well as corporate environments by providing flu shots, nutrition education, blood pressure and other health screenings, and the free loan of infant and sick room equipment (such as walkers, commodes and wheelchairs). Clinic services include: flu shots, well child and immunizations, foot care, blood pressure screenings, diabetic screenings.

Clinch River Home Health is licensed by the State of Tennessee as a home health agency.

Our services are often covered by Medicare, TennCare, or private insurance policies. However, if your condition does not qualify you for coverage by these insurance policies, you may opt to pay for the services at a fee which is based on your income.
